Facet Testimonial Release Agreement
Thank you for agreeing to share your experience as a Member(s) of Facet Wealth, Inc (“Facet”). We’re excited to work with you.
This Member(s) Testimonial Release Agreement (“Agreement”) is made and entered into as of 05/30/2025 by and between Facet and (“the Member(s)”).
The nature of the term of this Agreement will be ongoing and will cover all testimonials created by the Member(s).
Member(s) will be providing testimonials in the form of videos, quotes, or images, etc. to share his/her/their experience working with Facet. The content will be factual and compliant with the terms and conditions of the social media platforms used.
Member(s) is not being compensated for content created and distributed by Facet.
Member(s) consents to the use of his/her/their name (first name and last initial only), city and state of residence, voice, image, likeness, and any and all attributes of their personality, in, on or in connection with any film, audio tape, video tape, audio-visual work, photograph, illustration, animation, or broadcast, in any media or embodiment, now known or unknown, including, without limitation, all formats of computer readable media, produced by or for the benefit of Facet. Facet will not use any personally identifiable information beyond what is listed above.
Member(s) further consents to the use of his/her/their name, voice, image, likeness, and any and all attributes of his/her/their personality in any advertising or promotional material created or used in connection with Facet and each such item of advertising or promotional material will be considered a “Work” for purposes of this agreement. Member(s) acknowledges and agrees that the Work is made for Facet and belongs to and shall be the sole and exclusive property of Facet.
Member(s) hereby releases, discharges and agrees to hold harmless Facet, its legal representatives and assigns, all persons acting under its authority, and those for whom it is acting from all claims, causes of action and liability of any kind, now known or unknown, in law or equity, based upon or arising out of the video production or this agreement including, without limitation, claims of libel, slander, invasion of privacy, right of publicity, defamation, trademark infringement, and copyright infringement.
Member(s) represents and warrants to Facet that he/she/they have the power and authority to authorize this Agreement and that the testimonial will not violate the intellectual property rights of any third party.
Member(s) has read the above authorization, release and agreement, prior to its execution; and fully understands the contents thereof.
The nature of the term of this Agreement will be ongoing and will cover all testimonials created by the Member(s). Member(s) may revoke the rights to use their provided testimonial at any time by emailing [email protected].
